APPOINTMENT CHECKLIST

Verify that your doctors and medications are covered. Before your appointment with Hartman Insurance, we encourage you to jot down the doctors you see and the prescriptions you take so your agent can verify that they are covered by the plan. Click the button below to download your checklist.

To comply with updated Medicare regulations, a signed Scope of Appointment form is required prior to the start of your appointment. You may complete the applicable form(s) below and return them to INFO@HartmanInsuranceServices.com before your scheduled meeting. If you prefer not to sign electronically, you may sign the form at the beginning of your appointment. Please call our office with any questions.
